Rebar (steel reinforcement bar) in concrete, in some circumstances, can rust. The rust product occupies more volume than steel and breaks up the concrete.
It is important to break out all loose concrete and expose all the rusting rebar to then determine whether the rebar should be replaced or treated with a protective coating. After the rebar is restored then the concrete can be patched back.
Another reason concrete can break is from Not enough reinforcement or reinforcement not properly placed. In this case the steel reinforcement needs to be adjusted before the concrete is patched back.
